Después de un infarto de miocardio, la recurrencia de eventos coronarios continúa siendo un problema relevante. Por ello, la revascularización multivaso completa, guiada por angiografía o fisiología, tiene actualmente un nivel máximo de recomendación y evidencia.
Sin embargo, es posible que cuando guiamos por fisiología dejemos escapar algunas lesiones con capacidad de inestabilización en el seguimiento. Dentro de las posibles causas para explicar esta potencial limitación de la fisiología está el concepto de placa vulnerable. Pero ¿qué sabemos realmente sobre su fisiopatología? ¿Cómo debería influir en nuestra toma de decisiones clínicas?
